About Our Role

This includes assessing patient needs, documenting case activities, and collaborating with our multi-disciplinary team to connect patients with community resources.

Job Description

Key Responsibilities:

  • Interview patients, assess priorities, and document case activity.
  • Consult with members of the multi-disciplinary team to develop effective treatment plans.
  • Assist patients/families with obtaining community assistance by referral to proper resources.
  • Develop and maintain working relationships with community health, welfare, and social agencies.

Required Skills and Qualifications:

  • Master's degree in Social Work.
  • No experience required.
