Clinical Social Worker and Community Liaison

We are seeking a skilled Clinical Social Worker and Community Liaison to join our team at SSM Health. As a key member of our multidisciplinary team, you will play a vital role in providing social work assistance to patients and families, ensuring their safe hospital discharges and identifying any barriers that may impact their treatment.
About the Role:This exciting opportunity is for a dedicated individual who is passionate about delivering exceptional patient care and making a positive impact on the lives of others. As a Clinical Social Worker and Community Liaison, you will be responsible for interviewing patients, assessing priorities, and documenting case activity. You will also consult with members of the multi-disciplinary team to ensure seamless communication and collaboration with community resources to support patient treatment.
Main Responsibilities:- Patient Interviews and Assessments: Conduct thorough interviews with patients to assess their needs and develop effective plans to address them.
- Community Resource Referrals: Assist patients and families in accessing community resources and services to support their recovery and well-being.
- Collaboration with Healthcare Teams: Work closely with healthcare teams to identify opportunities for improvement in standard work and contribute to enhancing patient care.
To be successful in this role, you will need to have a Master's degree in Social Work and excellent communication and interpersonal skills. Experience in social work or a related field is an asset.

- $70,000 - $90,000 per annum (estimated salary)
