About the Role

The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their residences, which may include private homes, skilled nursing facilities, and assisted living facilities. As a key member of the interdisciplinary team, the RN will provide expertise on psych-social assessments, interventions, and community resources. This role can be based in Case Management, Admissions, or Resource.

Requirements
  • Current RN licensure in good standing in the state(s) of practice
  • Ability to work effectively within a team environment, collaborating with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Excellent communication and time management skills to prioritize tasks, manage visits, and document patient information
  • Ability to learn and utilize an electronic medical records system
  • Reliable transportation, valid driver's license, and auto insurance
