Senior Medical Care Provider

Abode Care Partners is seeking a compassionate and skilled Nurse Practitioner (APRN) to deliver high-quality medical care to seniors and patients with complex needs. As a key member of our interdisciplinary team, you will have the opportunity to make a meaningful difference in the lives of those we serve.
Responsibilities
- Medical Care Delivery: Provide evidence-based medical care to patients, including chronic and urgent episodic illnesses, minor injuries, and other medical conditions.
- Collaboration: Work closely with caregivers and other healthcare professionals to develop and implement comprehensive plans of care that meet the unique needs of each patient.
- Patient Advocacy: Ensure that patients receive personalized care and attention, addressing their physical, emotional, and social needs.
- Communication: Maintain open and effective communication with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
- Licensure: Active state Nurse Practitioners license and current Nurse Practitioner National Board Certification are required.
- Experience: A minimum of five years of nursing experience, with at least two years of nurse practitioner experience preferred.
- Skills: Excellent interpersonal and communication skills, strong time management abilities, and proficiency in medical instruments and equipment.
Compensation and Benefits
The salary for this position is estimated to be between $100,000 and $130,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including competitive compensation, flexible scheduling, mileage reimbursement, and paid time off.
