Complex Care Social Worker Opportunity

We are seeking a skilled Complex Care Social Worker to join our team at Grace Medical Center. As a key member of our interdisciplinary team, you will play a vital role in facilitating discharge planning for patients with complex needs.

Job Summary:

The Complex Care Social Worker will work closely with patients, families, and healthcare providers to identify and address barriers to discharge. Using a systematic approach, you will gather pertinent information, develop and implement plans of care, and collaborate with the team to ensure effective management of care and resource utilization.

Responsibilities:

  • Conduct thorough assessments of patients' needs and develop individualized plans of care.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective resource utilization.
  • Provide education and support to patients and families to promote self-advocacy and empowerment.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.

Requirements:

  • Master's degree in Social Work or related field.
  • Current licensure as a LCSW-C or LCPC in the State of Maryland.
  • Minimum of 3 years of experience in social work or a related field.
