Complex Care Social Worker - Renal Dialysis Opportunity

We are seeking a skilled Complex Care Social Worker to join our team at Grace Medical Center. As a key member of our interdisciplinary team, you will play a vital role in facilitating discharge planning and ensuring timely and cost-effective care for our patients.

Job Summary:

The Complex Care Social Worker will be responsible for identifying patients with barriers to discharge and working collaboratively with the interdisciplinary team to develop and implement a plan of care. This will involve gathering pertinent information, overseeing the efficient management of care, and communicating effectively with patients, care team members, and community partners.

Key Responsibilities:

  • Systematically gather pertinent information to identify patient needs and develop effective care plans.
  • Oversee the efficient management of care and ensure appropriate utilization of resources.
  • Collaborate with the interdisciplinary team to develop and implement care plans.
  • Communicate effectively with patients, care team members, and community partners.

Requirements:

  • Master's degree in social work or a related field.
  • Current/Valid license in the State of Maryland; LCSW-C/LCPC required.
  • Minimum of 3 years of experience; 5+ years of experience preferred.
