Renal Dialysis Social Worker - LifeBridge Health

Job Summary:

We are seeking a skilled Renal Dialysis Social Worker to join our team at LifeBridge Health. As a key member of our interdisciplinary team, you will play a critical role in facilitating discharge planning for patients with complex needs.

Key Responsibilities:

  • Develop and implement effective discharge plans in collaboration with the interdisciplinary team.
  • Identify and address barriers to discharge, including social determinants of health and access to community resources.
  • Work closely with patients, families, and caregivers to ensure a smooth transition from hospital to home or other care settings.
  • Collaborate with healthcare providers to ensure timely and effective care coordination.
  • Provide education and support to patients and families on disease management, treatment options, and community resources.

Requirements:

  • Master's degree in Social Work (MSW) or related field.
  • Current licensure as a Licensed Certified Social Worker-Clinical (LCSW-C) or Licensed Clinical Professional Counselor (LCPC) in the State of Maryland.
  • Minimum of 3 years of experience in social work, with a focus on renal dialysis or related field.
