We are seeking a skilled Complex Care Social Worker to join our team at Grace Medical Center. As a key member of our interdisciplinary team, you will play a critical role in facilitating discharge planning and ensuring timely and cost-effective care for our patients.

Key Responsibilities:

  • Develop and implement effective discharge plans in collaboration with the interdisciplinary team.
  • Identify and address barriers to discharge, utilizing a multi-step escalation protocol.
  • Monitor high-cost and difficult-to-place patients to identify early intervention opportunities.
  • Collaborate with the care team to develop and implement a plan of care.
  • Communicate effectively with patients, care team members, and community partners.

Requirements:

  • Master's degree in social work or related field.
  • Current/Valid license in the State of Maryland; LCSW-C/LCPC required.
  • Minimum of 3 years of experience; 5+ years of experience preferred.
