As a Medical Social Worker at CommonSpirit Health at Home, you will play a vital role in supporting patients and enhancing their lives. You will work closely with our multidisciplinary team to develop personalized care plans that address the physical, emotional, and social needs of our patients.

About the Role

This is a challenging yet rewarding role that requires strong communication and interpersonal skills. You will work closely with patients, family members, and healthcare professionals to understand their needs and develop effective care plans. Your expertise will ensure that every patient receives comprehensive support, addressing their social, emotional, and economic challenges.

Responsibilities

- Develop personalized care plans in collaboration with the patient's healthcare team
- Provide emotional support and counseling to patients and their families
- Connect patients with vital community resources, enhancing their overall well-being

Requirements

- Master's degree from an accredited school approved by Council on Social Work Education is required
- Current licensure to practice in WA state is required
- One year of social work in a healthcare setting is preferred
- CPR certification is necessary
