Your Responsibilities:

Your role will focus on leveraging your expertise to assist members in making informed health decisions.

In this position, you will actively manage the experiences of prospective customers and members within a Florida Blue Center, assessing their needs, educating them on available benefit plans, costs, and resources, and making appropriate referrals to Florida Blue programs.


Sources of contacts and referrals may include member self-service, Care Managers, the member contact center, and various community outreach initiatives.

You will also conduct health education sessions and Personal Health Assessments, providing valuable insights to members on achieving better health outcomes.

Key Information:

  • This position is based at our Florida Blue Center.

Operational Hours:
Monday - Friday 9:00 a.m. - 7:00 p.m. Saturday 9:00 a.m. - 4:00 p.m.

The essential functions of this role include:

  • Conducting Personal Health Assessments and interpreting biometric results
  • Addressing member care gaps through accurate documentation
  • Delivering health education sessions
  • Assessing prospective customer and member needs to ensure effective service delivery
  • Collaborating with teams to enhance Enterprise STARS ratings through targeted initiatives
  • Engaging in outreach campaigns for various member groups
  • Educating members on benefit plan options and implications
  • Resolving access to care issues and facilitating problem-solving
  • Connecting members with community resources
  • Guiding members through benefits structure and care alternatives
  • Providing referrals to corporate programs
  • Coordinating personalized care plans for members
  • Hosting virtual customer appointments
  • Educating members on digital health platforms

Qualifications:

Experience:

  • Minimum of 1 year of nursing experience
  • Bilingual in Spanish and English
  • Understanding of benefit plans
  • Flexibility to work varied hours, including evenings
  • Knowledge of the healthcare continuum
  • Experience with automated medical systems
  • Health coaching experience

Licenses and Certifications:

  • Active RN license in Florida or Compact State Licensure
  • Certified Health Coach within 180 days
  • BCLS - Basic Life Support CPR Certification upon hire

Education:

  • Bachelor's degree in a related field, preferably Social Work

Preferred Qualifications:

  • 3+ years of experience in managed care, case management, or clinical nursing
  • Sales experience
  • Knowledge of provider contracting and reimbursement
  • Familiarity with member tools and care management programs
  • Understanding of community resources
