POSITION SUMMARY

The Client Services Coordinator plays a crucial role in managing the intake and referral processes to ensure clients receive appropriate services. This position involves coordinating in-home care requirements while adhering to Federal and State regulations, as well as Managed Care Organization (MCO) guidelines. The coordinator will oversee a caseload of clients enrolled in Medicaid programs and perform regular assessments to adjust care plans based on evolving client needs.

KEY RESPONSIBILITIES

  • Oversee and manage a caseload of clients enrolled in Medicaid programs.
  • Ensure compliance with Federal and State regulations and MCO program rules while coordinating clients' in-home care needs.
  • Facilitate daily intake and referral processes to support business growth and development.
  • Foster a collaborative and supportive office environment.
  • Assist in daily operations and advocate for clients' needs.
  • Work independently with minimal supervision to meet job requirements.
  • Engage with stakeholders to ensure effective service delivery.
  • Exhibit strong written and verbal communication skills.
  • Adhere to legal requirements, standards, policies, and procedures.
  • Demonstrate reliability and accountability in all tasks.
  • Utilize effective problem-solving and decision-making abilities.
  • Show proficiency in computer applications relevant to the role.
  • Manage complex service programs and special projects as assigned.
  • Promote services to referral sources as necessary.
  • Maintain the skills and knowledge required to effectively coordinate workflow.
  • Participate in ongoing professional development and training.
  • Effectively prioritize and manage multiple tasks.
  • Deliver exceptional customer service to both internal and external clients.
  • Represent the organization at various meetings and events.
  • Assist clients and employees with enrollment in services and systems as needed.
  • Conduct necessary assessments and home visits as required.
  • Bilingual skills are preferred in various locations.
  • Handle phone and video communications as necessary.
  • Perform additional duties as assigned.

QUALIFICATIONS

  • A Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology, or a related field is preferred.
  • At least one year of experience working with individuals with disabilities or the elderly is preferred.
  • A combination of education and relevant experience will be considered.

The Client Services Coordinator typically operates in an office setting, utilizing a computer, telephone, and other office equipment to fulfill job responsibilities. The work environment is standard for an office, with potential for frequent interruptions. The role requires sitting, talking, and listening, as well as the ability to perform repetitive hand motions and lift up to 20 pounds.
