Job Type: Full-time

Description:

We are seeking a skilled Care Coordinator to join our team at Children's Home of Jefferson County. As a Care Coordinator, you will provide comprehensive and person-centered care coordination and care management services for members experiencing emotional and/or behavioral problems or those with complex combinations of chronic conditions/serious mental illnesses.

Key Responsibilities:

• Conduct strength-based assessments to identify individual member needs

• Develop and implement individualized service plans in collaboration with members, family members, and service providers

• Provide advocacy and support to individual members to ensure access to various community services

• Collaborate with all members of the care team to ensure seamless transitions and high-quality care

Work Environment:

This role requires working in an office setting and completing off-site/in-home visits. You will have contact with service recipients who may be agitated and/or confused. Some risk is involved working with service recipients with serious mental illnesses. You must possess visual and audio skills to complete job duties.

Requirements:

• An Associate's Degree in psychology, sociology, human services, or a related field is required

• A Bachelor's Degree is strongly preferred

• Valid NYS Driver's License with a good driving record

Salary:

Starting at $46,000
