Optimae Home Health is committed to delivering exceptional care to our patients. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in the Washington area. As an LPN, you will play a vital role in providing direct patient care under the supervision of a registered nurse. Your responsibilities will include following the plan of care, providing treatments, and collaborating with the healthcare team to achieve positive patient outcomes.

**Key Responsibilities:**

* Provide direct patient care as defined in the state Nurse Practice Act
* Implement the plan of care initiated by the registered nurse
* Document patient information accurately and in a timely manner
* Assess and educate patients and their families on diagnosis and care
* Assist registered nurses and physicians with specialized procedures
* Use equipment and supplies effectively and efficiently
* Participate in personal and professional growth and development

**Requirements:**

* Graduate of an accredited practical nurse or vocational nursing program
* Preferably 3 years of nursing experience, with community/home health or medical/surgical experience preferred
* Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
* Comply with accepted professional standards and practices
* Possess good verbal and written communication, and organization skills
* Maintain CPR certification
* Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
