As a Clinical Care Coordinator, you will engage with a multidisciplinary care team to support high-risk patients throughout their healthcare journey.

The Clinical Care Coordinator is responsible for managing care and disease management over time for patients with chronic conditions or those requiring surgical care. This role emphasizes patient outreach and care coordination for a designated patient panel to achieve optimal health outcomes, enhance wellness, reduce preventable emergency department visits, and improve patient satisfaction.

Key Responsibilities:

  • Identify patients within the specialty care practice who require ongoing care coordination for their specific conditions.
  • Define the nature and duration of involvement needed from the specialty care team and coordinator, and identify the primary care team involved.
  • Utilize assessment skills and risk assessment tools to pinpoint patients with actual or potential care needs necessitating coordination.
  • Conduct targeted outreach to a defined group of high-risk patients (chronic illnesses, lack of social support, readmissions, emergency department visits, surgical cases, etc.) to ensure timely and effective care delivery.
  • Employ technological tools (registries, patient lists, care team tabs, etc.) to manage patient populations.
  • Perform comprehensive clinical assessments that encompass disease-specific, age-specific, medical, behavioral, pharmacy, social, and end-of-life needs of each patient.
  • Inform patients and families about their care coordination and communicate this information with the healthcare team.
  • Collaborate with the interdisciplinary team to establish goals and develop intervention plans to optimize patient outcomes.
  • Monitor patient adherence to the care plan.
  • Conduct reassessments regarding patient progress toward goals and update care plans as necessary.
  • Ensure care gaps are addressed concerning specialty diseases, chronic conditions, and surgical episodes.
  • Act as the primary patient contact for the team regarding conditions or surgical episodes and facilitate access to services.
  • Coordinate the members of the patient care team.
  • Serve as a liaison between patients, families, and healthcare providers, advocating for patients and addressing their questions and concerns.
  • Assist in managing transitions of care across various settings, ensuring effective communication and planning. Identify barriers to care and facilitate solutions.
  • Partner with other care coordinator teams, including primary care, transitional care, social work, rehabilitation, pharmacy, and palliative care.
  • Define and ensure adherence to disease-specific care pathways for specialty care or chronic conditions.
  • Collaborate with patients and families to assess current knowledge, health literacy, and readiness to change, utilizing teach-back methods to evaluate understanding.
  • Coach patients and families on self-management support, including setting both long-term and short-term goals.
  • Educate patients about managing specialty or surgical conditions (including preoperative, perioperative, postoperative, and recovery phases) along with prevention and health maintenance tasks. Connect patients to other care providers and community resources to enhance care.
  • Collaborate with practices on quality and process improvement initiatives.
  • Perform other duties as assigned.

Education Requirements:

  • Graduate from an accredited nursing program.
  • BSN preferred.

Certifications Required:

  • Current state licensure as a Registered Nurse (RN).
  • Specialty certification preferred.
  • Basic Cardiac Life Support (BLS) required.
  • Registered nurses or advanced practice nurses must obtain cancer-specific certification or demonstrate ongoing qualifying education within the timeframe of the facility's accreditation cycle if working in oncology-related areas.

Work Experience:

  • Three to five years of nursing experience required.

Physical Requirements:

  • Requires full range of motion, manual dexterity, and eye-hand coordination.
  • Requires corrected hearing and vision within normal ranges.
  • May involve some exposure to communicable diseases or bodily fluids.
  • Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly.

Personal Protective Equipment:

  • Adhere to Standard Precautions using personal protective equipment as required for procedures.
