Clinical Care Coordinator - Full Time

This role involves working in collaboration with multidisciplinary teams throughout the care continuum to facilitate and ensure efficient transitional care management.

Key to this position are the fundamental values of patient advocacy, comprehensive assessment, strategic care planning, and empowering patients and their families to manage their own health effectively.

The Clinical Care Coordinator guarantees adherence to the standards set by accrediting organizations, state regulations, and the CMS Conditions of Participation.


  • Key Responsibilities:
  • Fosters collaboration among healthcare professionals during the acute care phase and ensures a seamless transition to the next level of care or community services to maintain continuity.
  • Proactively evaluates, identifies, and addresses potential barriers that may hinder patients' transitions of care.
  • Collaborates with patients and/or their support persons to create a tailored, comprehensive transitional care strategy. Participation and leadership in patient rounds, huddles, and relevant committee meetings are expected.

Qualifications:

  • Graduate from an accredited nursing program.
  • Bachelor of Science in Nursing (BSN) is preferred.

Certifications:

  • Current State Licensed Registered Nurse (RN).
  • Certification from an accredited body is preferred within one year of eligibility, e.g., CCTM, ACM, CCM. American Heart Association.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).

Competencies:

  • Requires strong critical thinking abilities, decisive judgment, and the capacity to work independently.
  • Must be capable of functioning in a high-pressure environment and taking appropriate actions.

Experience:

  • A minimum of three years of recent clinical nursing experience is required. At least two years of recent experience in Utilization Review, Quality, or Care Management is preferred.
  • Familiarity with acute care regulatory and accreditation standards is preferred. Experience in discharge planning and/or Quality Improvement is advantageous.

Physical Requirements:

  • Requires normal or corrected hearing and vision within the normal range.
  • Ability to perform work in a stationary position for extended periods.
  • Ability to communicate and exchange accurate information.
  • Ability to manage physical records, including retrieval and filing. Proficiency in operating a computer and other office equipment.
  • Ability to travel within the healthcare system.
  • In certain locations, the ability to lift up to 10 pounds may be required.
