The Clinical Care Coordinator ensures adherence to the standards set by accrediting organizations, state regulations, and CMS Conditions of Participation.


  • Enhances inter-professional collaboration during the acute care phase and ensures a seamless transition to the next level of care or community resources to maintain continuity of care.
  • Proactively evaluates, identifies, and addresses potential obstacles to patients' transitions of care.
  • Partners with patients and/or their caregivers to create a detailed, personalized transitional care strategy. Requires graduation from an accredited nursing program.
  • Demonstrates critical thinking abilities, sound judgment, and the capacity to work independently.
  • Work Experience:
  • Preferred minimum of two years of recent experience in Utilization Review, Quality Assurance, or Care Management.
  • Familiarity with acute care regulatory and accreditation standards is preferred. Experience in discharge planning and/or Quality Improvement is also preferred.

Proficient in operating a computer and other office equipment.


  • Willingness to travel within the healthcare system.
