The role of **Patient Care Liaison** is vital, emphasizing outstanding customer service and patient support.

Key responsibilities include engaging directly with patients and their families to facilitate, coordinate, and ensure the successful completion of specialty care appointments.

This involves collaborating with insurance representatives and external partners, arranging transportation, and coordinating with physicians, clinicians, and other healthcare professionals to ensure seamless referral processes.

PRIMARY RESPONSIBILITIES:

+ Serve as the primary contact for incoming and outgoing patient referrals. Evaluate referrals, gather necessary information, ensure timely processing, and assign to appropriate providers.+ Promote effective communication, teamwork, and care coordination. Organize appointments, referrals, and transitions of care among primary care providers, specialists, hospitals, and other healthcare facilities, ensuring smooth transitions and continuous care.
  • Manage patient schedules using the coordinated provider list (CPL), handle all appointment-related logistics, and inform patients about appointment details:

date, time, and location.+ Utilize online insurance platforms to generate referral authorizations.+ Clearly communicate physician/clinician requirements or outstanding tasks to patients.+ Monitor all referrals until appointment completion, gather all relevant documentation, and upload to the organization's medical record system for physician review prior to follow-up with the primary care provider.

Ensure rescheduling of any missed external appointments and notify the primary care provider accordingly.+ Address referral-related inquiries from patients, providers, and other stakeholders.

Respond to and manage phone messages in a timely manner.+ Deliver exceptional customer service to all internal and external clients.+ Perform additional assigned duties as needed.

SKILLS AND QUALIFICATIONS:

+ Familiarity with medical terminology, CPT, HCPCS, and ICD coding is advantageous.+ Understanding of the patient demographic, including the complexities of Medicare programs.+ Exceptional organizational skills, with the ability to prioritize tasks and meet deadlines.+ Detail-oriented and capable of multitasking effectively.+ Proficient phone etiquette and communication skills.+ Ability to navigate various software systems and utilize technology efficiently.+ Excellent interpersonal skills with patients, colleagues, physicians, and other professionals in a respectful manner.+ Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, and Outlook), database management, and presentation software.+ Fluent in spoken and written English; bilingualism is a plus.

EDUCATIONAL BACKGROUND AND EXPERIENCE:

+ High School diploma or equivalent is required.+ Minimum of 1 year of experience in a healthcare referral setting is necessary.+ Familiarity with online insurance portals and obtaining referrals/authorizations from multiple payors is preferred.+ Experience in healthcare within the Medicare Advantage sector is a plus.+ Certification as a Medical Assistant is beneficial.+ CPR certification for Healthcare Providers is an added advantage.
