Role Overview

The Medication Management Specialist will play a vital role in implementing Su Vida's healthcare model to enhance the provider-patient relationship and engagement by improving the quality and affordability of care. This individual will focus on supporting providers and patients through medication management services, pharmacist-led clinical initiatives, and practice enhancement. You will be responsible for optimizing medication therapy to improve clinical outcomes such as blood pressure and A1c control, minimize preventable emergency department visits and hospitalizations, and monitor adherence to medication regimens and care plans. The specialist will meet patients where their needs are (virtually, in-office, and at-home) to identify, resolve, and prevent medication-related issues.

Key Responsibilities
  • Oversee pharmacist consultations for chronic care management of specific disease states, post-discharge medication management, and other consults
  • Utilize various technology platforms (Carium, Elation, etc.) to manage ongoing care for patients enrolled in the program
  • Conduct comprehensive medication evaluations while navigating multiple sources of information (EMR, pharmacy, patient, etc.) and ultimately create and follow up on a care plan with the patient and other care teams as well as the provider team
  • Assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns
  • Collaborate with providers, interdisciplinary care teams, patients, and/or caregivers to establish patient-specific therapy goals
  • Deliver patient-centered education on chronic disease states, including medications used for prevention and treatment
  • Provide patient care services through provider-delegated collaborative practice agreements (e.g., refill approvals, therapeutic interchange, disease state management)
  • Assist in the development and presentation of educational materials for patients and providers
  • Engage in quality improvement programs and initiatives driven by clinical needs
  • Maintain a comprehensive knowledge base on all health plan pharmacy benefits
  • Serve as a drug information resource to Su Vida Services Inc's care team and patients
  • Participate in patient discussions as a member of the multidisciplinary team
  • Support and uphold the excellence of Su Vida's clinical programs - identifying staff and practice training opportunities to optimize the patient experience and quality of care
  • Ensure your team, peers, and patients always have a positive experience by providing a high level of service promptly, professionally, and empathetically
  • Demonstrate the ability to work independently, solve problems, and make sound decisions in a timely manner
  • Exhibit strong verbal and written communication skills, interpersonal relationship skills, and problem-solving abilities
Qualifications

Knowledge, Skills, and Abilities
  • Pharm D. degree or equivalent education in pharmacy
  • Active, unrestricted Pharmacy license in Texas, or the ability to obtain one
Education, Experience, Licensure, or Certification Requirements
  • 3+ years of clinical pharmacist experience or equivalent residency experience
  • Proficient in Microsoft Office applications including Word, Excel, and PowerPoint
  • Bilingual/Bicultural (English and Spanish)
