Key Responsibilities
  • Record Review: Conduct thorough reviews of patient records to evaluate documentation for accuracy and completeness, identify areas for improvement, and initiate physician queries as needed.
  • Documentation Improvement: Collaborate with healthcare professionals to educate them on documentation guidelines, best practices, and regulatory requirements.
  • Query Management: Formulate and manage physician queries to ensure accurate and compliant documentation, and resolve any discrepancies or issues that arise.
  • Training and Development: Assist in training new staff members on documentation guidelines, best practices, and regulatory requirements.
Requirements
  • Education: Bachelor's degree in Health Information Management, Nursing, or related field.
  • Experience: 2+ years of experience in clinical documentation improvement, medical records review, or related field.
  • Certifications: RHIT, RHIA, LPN, MD, AAPC, CCS, or CDIP/CCDS preferred.
