About the Role

Key Responsibilities
  • Conduct thorough assessments of members' social drivers of health and access to healthcare needs, in collaboration with an interdisciplinary case management team.
  • Collaborate with healthcare providers to support members' treatment plans and care needs.
  • Recognize members' right to self-determination and autonomy, including their right to make informed choices that may not align with targeted outcomes.
  • Assess members' health behaviors, cultural influences, and belief/value systems, and evaluate all information related to current/proposed treatment plans and clinical practice guidelines to identify potential barriers.
  • Manage and maintain a caseload of members with high severity and intensity of service needs, providing support and linkages to appropriate supportive services.
  • Coordinate services with state and community programs on behalf of the member, particularly when the member is unable to effectively do so independently.
  • Conduct field visits in various settings, including hospitals, skilled nursing facilities, and private residences, to complete assessments and provide psychosocial interventions.
  • Document psychosocial assessments and referrals thoroughly and in a timely manner, according to established standards and guidelines.
  • Actively participate in interdisciplinary team meetings and case consultations.
Requirements
  • Master's Degree in Social Work (MSW) or related field from an accredited university.
  • Licensed Clinical Social Worker (LCSW) or equivalent licensure (e.g. LPCC, LMFT, etc.) preferred.
  • Case Manager Certification (CCM) preferred.
  • 3 or more years of experience working with the Medi-Cal population, medical social work, community clinics, or other home and community-based organizations.
  • Strong analytical and problem-solving skills.
  • Experience conducting thorough psychosocial assessments, developing care plans, and providing necessary interventions with complex client populations.
  • Thorough knowledge of services provided by, and eligibility for, applicable state and federal programs, including Long Term Services and Supports (LTSS), in addition to local, community-based organizations.
  • Valid CA Driver's License and proof of auto insurance coverage required.
  • Proficient skills with computer programs such as Microsoft Excel, Outlook, Word, and PowerPoint.
  • Excellent communication skills, both orally and in writing.
  • Knowledge of health disparities in diverse communities.
  • Effective time management skills and ability to define and act on priorities amid multiple responsibilities.
  • Bilingual Spanish speaking preferred.
