Social Services Manager

We are seeking a highly skilled and experienced Social Worker to join our team at Blue Shield of California. As a Senior Field-Based Social Worker, you will play a critical role in providing social services to our members and helping them navigate the healthcare system.
Key Responsibilities- Conduct thorough assessments of members' social drivers of health and access to healthcare needs, in collaboration with an interdisciplinary case management team.
- Collaborate with healthcare providers to support members' treatment plans and care needs.
- Recognize members' right to self-determination and autonomy, including their right to make informed choices that may not align with targeted outcomes.
- Assess members' health behaviors, cultural influences, and belief/value systems, and evaluate all information related to current/proposed treatment plans and clinical practice guidelines to identify potential barriers.
- Manage and maintain a caseload of members with high severity and intensity of service needs, providing support and linkages to appropriate supportive services.
- Coordinate services with state and community programs on behalf of the member, particularly when the member is unable to effectively do so independently.
- Conduct field visits in various settings, including hospitals, skilled nursing facilities, and private residences, to complete assessments and provide psychosocial interventions.
- Document psychosocial assessments and referrals thoroughly and in a timely manner, according to established standards and guidelines.
- Actively participate in interdisciplinary team meetings and case consultations.
- Master's Degree in Social Work (MSW) or related field from an accredited university.
- Licensed Clinical Social Worker (LCSW) or equivalent licensure (e.g. LPCC, LMFT, etc.) preferred.
- Case Manager Certification (CCM) preferred.
- 3 or more years of experience working with the Medi-Cal population, medical social work, community clinics, or other home and community-based organizations.
- Strong analytical and problem-solving skills.
- Experience conducting thorough psychosocial assessments, developing care plans, and providing necessary interventions with complex client populations.
- Thorough knowledge of services provided by, and eligibility for, applicable state and federal programs, including Long Term Services and Supports (LTSS), in addition to local, community-based organizations.
- Valid CA Driver's License and proof of auto insurance coverage required.
- Proficient skills with computer programs such as Microsoft Excel, Outlook, Word, and PowerPoint.
- Excellent communication skills, both orally and in writing.
- Knowledge of health disparities in diverse communities.
- Effective time management skills and ability to define and act on priorities amid multiple responsibilities.
- Bilingual Spanish speaking preferred.
