As a Home Health Clinical Manager, you will play a vital role in shaping the care experience for our patients. You will oversee the delivery of high-quality, patient-centered care, ensuring that our patients receive the best possible outcomes. About the Role

Key Responsibilities:

  • Develop and implement clinical strategies to improve patient outcomes and satisfaction.
  • Lead a team of healthcare professionals in providing high-quality, patient-centered care.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Monitor and evaluate the quality of care delivered, identifying areas for improvement and implementing changes as needed.
  • Develop and maintain relationships with referral sources, community partners, and other stakeholders to promote our services and build partnerships.
  • Stay up-to-date with industry trends, best practices, and regulatory requirements, applying this knowledge to improve our services and stay ahead of the competition.
Requirements

Essential Qualifications:

  • Graduate of an accredited School of Nursing.
  • BSN Required, NY State requirements.
  • Current state license as a Registered Nurse.
  • 2 years of Home Health experience with a CHHA required.
  • Proof of current CPR.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice, or equivalent environment.
