About the Role:

As a Clinical RN Manager at CenterWell Home Health, you will play a vital role in delivering exceptional patient care and leading a team of dedicated healthcare professionals. Your expertise and leadership skills will enable you to coordinate and oversee direct care patient services, ensuring the highest quality of care for our patients.

Key Responsibilities:

  • Develop and implement clinical operations for a specific location, ensuring seamless patient care and staff management.
  • Conduct assessments and reassessments of patients, updating care plans and interpreting patient needs to ensure optimal care delivery.
  • Manage caregiver assignments, ensuring the right staff are in place to meet patient needs.
  • Oversee the delivery of care to all patients, receiving case referrals and reviewing patient information to determine home health or hospice needs.
  • Instruct and guide clinicians to promote effective performance and delivery of quality home care services.
  • Assist clinicians in establishing therapeutic goals, setting priorities, and developing patient Plans of Care (POC).
  • Monitor cases to ensure documentation compliance with regulatory agencies and third-party payers.
  • Coordinate communication between team members, attending physicians, and caregivers to ensure appropriateness of care and outcome planning.
  • Work with the Branch Director and Company Finance Department to establish location revenue and budget goals.
  • Participate in sales and marketing initiatives to promote our services.
  • Supervise clinical employees, responsible for the overall direction, coordination, and evaluation of the location.
  • Handle employee corrective action and discipline issues fairly and objectively.
  • Participate in interviewing, hiring, training, and development of direct care clinicians.
  • Analyze situations, identify problems, and evaluate alternative courses of action using Performance Improvement principles.
  • Responsible for reviewing case manager and clinical staff documentation for appropriateness of care, delivery, and documentation requirements.
  • Perform QA/PI activities, working with Utilization Review staff to track performance and outcomes of care analysis.
  • Act as Branch Director in their absence.
  • Interpret Company standards and policies to ensure compliance with external regulatory authorities.
  • Participate in performance improvement activities, maintaining ongoing clinical knowledge through internal and external training programs.
  • Maintain relationships with referral/community sources, participating in professional organizations and conducting care-related programs.

Requirements:

  • Graduate of an accredited School of Nursing.
  • BSN Required, NY State requirements.
  • Current state license as a Registered Nurse.
  • 2 years of Home Health experience with a CHHA required.
  • Proof of current CPR.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice, or equivalent environment.

Benefits:

CenterWell Home Health offers a competitive compensation package, including a base salary range of $76,800 - $105,800 per year, plus a bonus incentive plan based on company and individual performance.
