Key Responsibilities
  • Lead the Patient Navigator team to achieve high-quality patient care and outcomes.
  • Develop and implement training programs for new Patient Navigators to ensure they have the necessary skills and knowledge to provide excellent patient care.
  • Provide ongoing support and guidance to Patient Navigators to ensure they have the resources and information needed to provide high-quality care.
  • Oversee the auditing process for discount applications to ensure accuracy and timeliness.
  • Identify trends in audit errors and provide feedback to office managers to improve errors.
  • Track and analyze data on assistance provided to patients, including food, housing, and insurance assistance.
  • Assess patient needs based on reporting measures and communicate with supervisors to ensure effective care.
Requirements
  • High School Diploma or equivalent required.
  • Community Health Worker Certification required or obtained within the first year of employment.
  • Bachelor's degree in healthcare or social services related field, preferred.
  • Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private health insurance, SNAP, and other local, state, or national social safety net programs strongly preferred.
  • Ability to be certified as a Certified Application Counselor (CAC) and Senior's Health Insurance Information Program (SHIIP) counselor within one month of hire.
  • Valid driver's license and reliable transportation required.
