Lead Patient Navigator

Blue Ridge Health is seeking a highly skilled Lead Patient Navigator to join our Clinical Team in Columbus, NC. As a key member of our interdisciplinary team, you will play a vital role in improving patient outcomes and providing exceptional care.

Responsibilities:
  • Lead the Patient Navigator team to ensure seamless patient care and navigation through the healthcare system.
  • Develop and implement training programs for new Patient Navigators, ensuring they have the necessary skills and knowledge to excel in their roles.
  • Provide ongoing support and guidance to Patient Navigators, communicating changes in processes and procedures to ensure they are up-to-date and informed.
  • Assist during open enrollment periods to ensure timely completion of training and knowledge acquisition.
  • Oversee the auditing process for discount applications, ensuring accuracy and timeliness.
  • Identify trends in audit errors and provide feedback to office managers to improve efficiency.
  • Track and report assistance provided to patients, ensuring data is accurate and up-to-date.
Requirements:
  • High School Diploma or equivalent required.
  • Community Health Worker Certification required or obtained within the first year of employment.
  • Bachelor's degree in healthcare or social services-related field preferred.
  • Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private health insurance, SNAP, and other local, state, or national social safety net programs strongly preferred.
  • Ability to be certified as a Certified Application Counselor (CAC) and Seniors Health Insurance Information Program (SHIIP) counselor within one month of hire.
  • Valid driver's license and reliable transportation required.
