About the Role

As a Nurse Care Navigator, you will be responsible for conducting comprehensive assessments, developing care plans, and providing ongoing support to patients and their families.

Key Responsibilities
  • Conduct biopsychosocial assessments to identify patient needs and develop individualized care plans.
  • Collaborate with multidisciplinary teams to coordinate care and services for patients.
  • Provide education and support to patients and their families on health-related topics and resources.
  • Document patient progress and outcomes, participating in quality improvement initiatives.
Requirements

To be successful in this role, you will need:

  • A valid RN license and a minimum of two years of nursing experience.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Familiarity with community resources and healthcare systems.
Benefits

We offer a competitive salary range of $65,000 - $85,000 annually, depending on experience, plus benefits and opportunities for growth and development.
