Nurse Practitioner Role at Stout Street Recuperative Care Center

As a registered nurse at our Stout Street Recuperative Care Center, you will provide high-quality nursing care to program participants, adhering to accepted community standards of quality healthcare. Your expertise will also ensure continuity of care by collaborating with local hospitals, community clinics, social service agencies, and other CCH programs.
Responsibilities:
- Deliver exceptional patient care, supporting participants at Stout Street Recuperative Care Center.
- Collaborate with local healthcare organizations to guarantee seamless care transitions.
- Triage referrals based on patient acuity, program requirements, and bed availability.
- Provide tailored patient education, acknowledging individual learning styles, education levels, and cultural sensitivities.
- Promote patient-centered care within an integrated, trauma-informed, harm-reduction model.
- Empower patients by offering relevant health education and information.
- Work closely with the multidisciplinary healthcare team to achieve optimal patient outcomes.
- Be available to work weekends and holidays as scheduled or required.
- Document all clinical services in a timely and complete manner within the electronic health record.
- Maintain up-to-date records within the EHR.
- Record telephone communications and other clinical interactions as needed within the EHR.
- Manage tasks, PAQ, and scanning as necessary to ensure a complete and current chart.
- Keep accurate information regarding patient insurance status and complete CCARs/DACODs, and other clinical authorization documentation as needed.
- Utilize the EHR and any future upgrades as directed.
- Perform other duties as assigned.
Requirements:
- Associate Degree, Bachelor of Science in Nursing, or Master of Science in Nursing from an accredited institution.
- Current licensure from the State of Colorado to practice as a Registered Nurse.
- Certification in Basic Life Support.
- Successful completion of the privileging and credentialing process for RNs at the Coalition.
- Preferred experience in ambulatory, public, or community nursing.
- Desirable background in mental health, emergency room, urgent care, palliative care, care management, or critical care.
