Job Title: On-Call Overnight Registered Nurse

The Colorado Coalition for the Homeless is seeking a dedicated and compassionate On-Call Overnight Registered Nurse to join our team at the Recuperative Care Center. As a key member of our healthcare team, you will provide high-quality nursing care to patients in a fast-paced and dynamic environment.

Job Summary:
  • Provide acute and chronic nursing care to patients in the Recuperative Care program
  • Work collaboratively with local hospitals, community clinics, and social service agencies to ensure continuity of care
  • Triage and support program referrals based on patient acuity and program requirements
  • Develop and implement patient-centered care plans that address the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care and support
Responsibilities:
  • Provide nursing care to patients in the Recuperative Care program, including triage, assessment, and treatment
  • Work with local hospitals and community clinics to coordinate care plans and ensure continuity of care
  • Support program referrals and develop patient-centered care plans that address the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care and support
  • Document patient care and maintain accurate records in the electronic health record
Requirements:
  • AD, BSN, or MSN in nursing from an accredited college or university
  • Current licensure from the State of Colorado to practice as a Registered Nurse
  • Certification in Basic Life Support
  • Successful completion of privileging and credentialing process for RNs at the Coalition
  • Preferred experience in ambulatory, public, or community nursing
