Medical Records LPN

The Licensed Vocational Nurse (LVN) in the PACE Program provides direct and indirect patient care for adults 55 years of age or older with chronic or post-acute medical, cognitive, or mental health conditions. The LVN works under the supervision of a Registered Nurse (RN) and follows the established individualized plan of care for each participant.
The LVN evaluates home care needs, available support, and compliance with the developed Plan of Care. They seek to improve or stabilize deterioration of the medical, cognitive, and mental health condition, focusing on participants' lives and monitoring healthcare conditions while providing needed healthcare.
The LVN utilizes medical knowledge and training to identify and determine observations and changes in conditions that are essential to be reported promptly to the RN and/or primary care physician.
Key responsibilities include performing designated duties in accordance with the LVN scope of practice and established policies, procedures, guidelines, and techniques at both the Loma Linda University Medical Center PACE Center and in the place of the participant's residence if needed.
The LVN applies knowledge base in the delivery of quality whole-person care and demonstrates behaviors which support excellence in customer service and satisfaction, patient safety, and professionalism.
Requirements:
- Graduate of a school of vocational nursing
- Competencies in the fundamentals of patient care and knowledge of scope of practice for the licensed vocational nurse
- Basic Life Support (BLS) certification and First Aid certification issued by the American Heart Association
