Job Overview

POSITION SUMMARY: The role involves overseeing the delivery of essential services for individuals with Intellectual and Developmental Disabilities (IDD), ensuring they have access to necessary resources such as social, medical, educational, and vocational support.


QUALIFICATIONS: A Bachelor’s Degree in psychology, social work, or a related field in human services is required. Preference will be given to candidates with experience working with the IDD population.


SKILL SETS: Candidates should exhibit excellent organizational, verbal, and written communication abilities. Proficiency in Microsoft Office is essential, along with outstanding customer service skills. Dependability, effective interaction with the public and colleagues, adaptability to change, and a strong commitment to the role are crucial.


PHYSICAL AND ENVIRONMENTAL DEMANDS: The position requires frequent verbal communication and active listening when engaging with individuals, families, guardians, and team members. There may be exposure to communicable diseases and bloodborne pathogens. Visual acuity is necessary for assessing the condition of individuals and their surroundings. Home visits may present risks related to unsanitary conditions, abuse, or violence. The work involves light physical activity, including standing, sitting, and occasional bending. The ability to manage stress effectively and make informed decisions is essential.


KEY RESPONSIBILITIES:

  1. Maintain comprehensive documentation of monthly in-person visits, monitoring of services, and crisis interventions. Act as an advocate for individuals in accordance with relevant legislation, responding promptly to crisis calls and providing guidance on available options.
  2. Facilitate, review, and update Support Plans and Person-Centered Plans. Actively participate in the Support team that develops these plans for individuals receiving funding through various programs. Educate, train, and oversee the implementation of service plans, ensuring they incorporate principles of empowerment, community inclusion, and health and safety.
  3. Assist families and individuals in crisis situations that may necessitate facility admission to address their needs.
  4. Uphold the policy of human rights and participate in Human Rights Committee Meetings. Engage in the agency's continuous quality improvement initiatives.
  5. Perform additional duties as assigned.

MANDATORY REQUIREMENTS:

  2. Attend mandatory training and educational programs as required.
