Hospice Social Worker

We're seeking a compassionate and skilled Hospice Social Worker to join our team. As a vital member of our care team, you will provide emotional support to patients and their families during their end-of-life journey.
Key Responsibilities:
- Conduct psychosocial evaluations to understand patient needs and develop personalized plans of care.
- Provide ongoing counseling and support to patients and their families, addressing their emotional, social, and spiritual needs.
- Collaborate with our interdisciplinary team to ensure comprehensive care that addresses the physical, emotional, and spiritual well-being of patients.
- Document patient progress, identify areas for improvement, and contribute to quality improvement initiatives.
Qualifications:
- A Master's degree in Social Work (MSW) from an accredited school of social work approved by the Council on Social Work Education.
- A valid state license as a social worker.
- At least 2 years of clinical experience in healthcare or hospice, preferably in a similar role.
