**Job Summary:**

We are seeking a highly skilled Medical Social Services Coordinator to join our team at Alabama Hospice Care of Birmingham. As a key member of our interdisciplinary group, you will provide medical social services under the direction of a physician to assist patients and their families in understanding significant personal, emotional, environmental, and social factors related to their health status.

Responsibilities:

  • Assist patients, significant others, physicians, and healthcare team staff to understand significant personal, emotional, environmental, and social factors related to patient health status on an as-needed basis.
  • Contribute as a healthcare team member to the development of a comprehensive, integrated Plan of Care for patients daily.
  • Instruct healthcare team members on community resources available to assist patients as needed.

Requirements:

  • Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
  • One year of social work experience in a healthcare setting.
  • Current CPR certification required.
