Job Description

The Social Worker is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health and access to care. This role involves assessing patients for transition needs, identifying and addressing those at risk for readmission, and conducting complex psycho-social assessments and interventions to promote timely throughput and safe discharge.

This position integrates national standards for case management scope of services, including Transition Management, Care Coordination, Compliance with regulatory requirements, Education, and Leading a population of patients by service line.

Responsibilities
  • Coordination of complex discharge planning
  • Educating and mentoring new social work staff
  • Acting as a resource to all staff members regarding policies and procedures, as well as community resources
  • Trending barriers to care and resolving at least one barrier to care
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master of Social Work degree
  • At least 2 years of acute hospital experience
  • LCSW or LMSW licensure
