Senior Patient Care Manager

At Heartland Hospice, we are dedicated to providing exceptional care to our patients and their families. We are seeking a compassionate and experienced Senior Patient Care Manager to join our team.

About the Role
  • Supervise and manage a team of healthcare professionals, ensuring the provision of quality care to patients.
  • Oversee the day-to-day operations of a hospice site, administering the clinical aspects of the hospice program.
  • Manage and direct the services and performance of assigned team members.
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Degree in nursing (ASN or BSN) and one year of experience as an RN (licensed in the state of employment).
  • Three years of experience in direct patient care and one year of management experience.
  • Two or more years of experience in hospice or home care.
  • Ability to effectively operate nursing assessment tools and office equipment.
  • Sensitivity to the feelings of the terminally ill and their families.
Licenses and Certifications
  • Current license to practice nursing in the state of employment.
  • CPR certification.
  • Valid driver's license and current automobile insurance.
Personal Qualities and Aptitudes
  • Sensitivity to the feelings of the terminally ill and their families.
  • Ability to demonstrate flexibility and perform a variety of tasks.
  • Effective communication and interpersonal skills.
  • Ability to influence people and make generalizations and evaluations.
Specialized Knowledge and Skills
  • Knowledge of terminally ill patients and their families, as well as the hospice concept.
  • Excellent patient assessment skills and good oral and written communication.
  • Thorough knowledge of managed care principles, regulatory guidelines, and management principles.
