Senior Patient Care Manager

At Heartland Hospice, we are dedicated to providing exceptional care to our patients and their families. We are seeking a compassionate and experienced Senior Patient Care Manager to join our team.
About the Role- Supervise and manage a team of healthcare professionals, ensuring the provision of quality care to patients.
- Oversee the day-to-day operations of a hospice site, administering the clinical aspects of the hospice program.
- Manage and direct the services and performance of assigned team members.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
- Degree in nursing (ASN or BSN) and one year of experience as an RN (licensed in the state of employment).
- Three years of experience in direct patient care and one year of management experience.
- Two or more years of experience in hospice or home care.
- Ability to effectively operate nursing assessment tools and office equipment.
- Sensitivity to the feelings of the terminally ill and their families.
- Current license to practice nursing in the state of employment.
- CPR certification.
- Valid driver's license and current automobile insurance.
- Sensitivity to the feelings of the terminally ill and their families.
- Ability to demonstrate flexibility and perform a variety of tasks.
- Effective communication and interpersonal skills.
- Ability to influence people and make generalizations and evaluations.
- Knowledge of terminally ill patients and their families, as well as the hospice concept.
- Excellent patient assessment skills and good oral and written communication.
- Thorough knowledge of managed care principles, regulatory guidelines, and management principles.
