About the Role

The After Incarceration Support Program at Open Sky Community Services is seeking a highly motivated and compassionate individual to fill the position of Aftercare Specialist. As a key member of our team, you will play a vital role in supporting the re-entry of returning citizens and individuals with a history of justice system involvement.

Key Responsibilities
  • Provide oversight of the front desk, answering phone calls, and assisting with staff and space scheduling.
  • Manage office mail and deliveries, and provide crisis response supports to members as necessary.
  • Complete intakes and initial needs assessments for individuals, including gathering health insurance information and other eligibility requirements.
  • Assist with data entry for the program, including RedCap, EIM/ESM, ehana, and other systems.
  • Evaluate individuals' needs and service preferences, identifying current resources and services.
  • Complete all required documentation in a timely manner.
Qualifications
  • High School Diploma, GED, or equivalent, required.
  • Lived experience or minimum of 1 year experience working with people experiencing homelessness, justice involvement, serious and persistent mental illness, substance use disorder, or other underserved populations required.
  • Valid drivers license and acceptable driving record, required.
