Aftercare Specialist

We are seeking a highly motivated and compassionate individual to join our team as an Aftercare Specialist. In this role, you will play a critical part in supporting the re-entry of returning citizens who have been recently released from the Department of Corrections and other individuals who have a history of Justice System Involvement.

• Provide oversight of the front desk, answering phone calls, assisting with staff and space scheduling, and managing office mail and deliveries.
• Complete intakes and initial needs assessments for individuals, including gathering health insurance information and other eligibility requirements.
• Evaluate individuals' needs and service preferences while identifying current resources and services.
• Assist with data entry for the program, including RedCap, EIM/ESM, ehana, the triage tracker, and other systems.

• High School Diploma, GED, or equivalent, required.
• Lived experience or minimum of 1 year experience working with people experiencing homelessness, justice involvement, serious and persistent mental illness, substance use disorder, or other underserved populations required.
• Valid drivers license and acceptable driving record, required.
