Job Summary

Denali Family Services is seeking a highly skilled and compassionate Intake Coordinator to join our team. As the initial point of contact for referrals, you will play a critical role in coordinating the agency's intake process from start to finish.

Key Responsibilities
  • Intake and Admissions: Chair and facilitate the Intake meeting, screen potential youth and families for SED criteria, and gather treatment records from previous providers.
  • Referral Coordination: Coordinate foster care referrals and new placement updates, and maintain the Carelogic EHR database by entering all referrals and enrolling youth and families at time of intake.
  • Client Communication: Serve as the initial telephone contact for DFS referrals, generate applicable intake reports, and refer youth and families to other community resources as needed.
  • Documentation and Record-Keeping: Accurately document events, provide detailed records for youth and families, and maintain confidentiality as prescribed by agency policy and federal law.
Requirements
  • Education: Bachelor's degree from an accredited college or university in social work, human services, psychology, or a closely related field.
  • Experience: One year of experience in human services or a closely related field, with excellent written and oral communication skills, sound judgment, and therapeutic skills.
  • Skills and Abilities: Ability to maintain positive professional working relationships, work effectively within a team environment, and ensure youth, their family, and employee confidentiality.
Working Conditions

Work is performed in a community mental health center. Ability to perform sedentary work, visual acuity necessary to utilize a computer monitor, and ability to communicate verbally, both in person and over the telephone.
