Assessment Coordinator at The Salvation Army USA Western Territory

We are seeking a highly skilled Assessment Coordinator to join our team at The Salvation Army USA Western Territory. This is a full-time position responsible for providing initial introductions to services for Medicaid Waiver clients, following up with authorized clients, and coordinating with care coordination systems.
Key Responsibilities- Provide information on Older Alaskans Program services to clients and stakeholders
- Handle initial telephone intake for potential clients and refer them to other providers as needed
- Complete assessments to identify client needs and determine eligibility for services
- Coordinate with care coordinators, family members, and caregivers to determine client needs
- Provide client follow-up as needed and arrange for home visits when necessary
- Maintain accurate client case notes and files, and monitor waiting lists
- Assist in planning and carrying out targeted outreach efforts
- Promptly report any incidents of suspected or known elder abuse
- Associates degree in Human Services or equivalent related work experience
- Ability to accurately report functional, physical, social, and mental status of individuals
- Proficient in using a computer and applications, including Excel and Word
- Able to work independently with minimal supervision
- Current, valid Alaska driver's license
This position is performed in a well-lit, heated, and ventilated office environment with minimal noise levels. The use of personal protective equipment may be necessary during home visits.
Language SkillsThe successful candidate must be able to understand, follow, and give both written and oral instructions, and use speech and hearing for ordinary and telephone conversation.
Mathematical SkillsThe candidate must be able to perform simple calculations and apply concepts such as ratios and percentages in practical situations.
Performance SkillsThe ideal candidate will be creative, self-motivated, organized, flexible, and possess leadership skills.
