Job Title: Residential Services Coordinator

Job Summary:

We are seeking a highly motivated and compassionate Residential Services Coordinator to join our team at Hope Cooperative (aka TLCS, Inc.). As a key member of our staff, you will work closely with residents who are progressing toward independent living, providing daily support and guidance to help them achieve their goals.

Responsibilities:

  • Provide emotional support and demonstrate empathy to residents, promoting a sense of hope and recovery.
  • Observe, assess, and intervene in situations related to mental health symptoms, physical health symptoms, and substance use issues, referring clients to relevant resources as needed.
  • Develop and implement service plans with clients, meeting regularly to encourage progress and skill development.
  • Transport clients in your personal vehicle as necessary, providing rehabilitative services in the community and their homes.
  • Facilitate group activities, planning social, cultural, educational, and recreational events to promote socialization and skill-building.
  • Shop for food and household supplies to meet program or client needs, ensuring the office, house, rooms, units, and yard are clean and fit for occupancy.
  • Act as a Mandated Reporter, notifying authorities and intended victims of physical harm, and ensuring the protection and confidentiality of all Protected Health Information (PHI).

Requirements:

  • Ability to work with individuals who have a mental health diagnosis, with experience highly desirable.
  • Lived experience as a mental health consumer or a family member/loved one of a mental health consumer is desirable.
  • Mental health crisis intervention experience is highly desirable.
  • Must have work experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse).
  • Must have knowledge of 12-Step Programs and/or personal experience with substance abuse recovery.
