Salary: The estimated salary for this role is $40,000-$60,000 per year, depending on experience, with the option of a $20,000 sign-on bonus for individuals who have not previously participated in this program.

Job Description: As a team member of our Senior Community Care (SCC) product, you will work with a team to provide care to patients at home, nursing homes, assisted living facilities, and senior housing. This life-changing work adds a layer of support to improve access to care.

Required Skills and Qualifications: To be successful in this role, you must have a graduate degree in Nursing, active and unrestricted licensure as a Nurse Practitioner, and certification in one of the following areas: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner. You should also have current DEA licensure/prescriptive authority or the ability to obtain it post-hire.
