Job Summary:
Bay Cove Human Services is seeking a dedicated and compassionate Case Manager to support our mission in providing individualized and compassionate services to individuals facing challenges associated with addiction, mental illness, and homelessness.
Key Responsibilities:
- Arrange aftercare interviews and coordinate treatment services with external providers
- Provide counseling and support to clients during detox and withdrawal
- Document interventions and progress in counseling notes
- Attend staff meetings and supervision sessions
- Advocate for clients and provide community support information
Requirements:
- One-year experience in human services, substance use, and mental health
- Understanding of computer basics
