Job Summary:
The RN Field Care Manager will provide specialty service care management to beneficiaries, collaborating with care teams to ensure continuity of care across services.
Key Responsibilities:
- Develop and implement comprehensive care plans, encompassing all services and plans related to beneficiaries.
- Provide case management complying with Conflict Free Case Management rules.
- Conduct home visits, hospital visits, and assisted living facility visits to provide care management to members.
- Document care management activities and updates in member charts.
Requirements:
- Active RN license in the state of Texas.
- 1+ years of Care/Case Management experience in a managed care setting.
- Field work experience with home visits.
Preferred Qualifications:
- Behavioral health or home health background.
- Proficient with Microsoft programs, Outlook, Excel.
- Medicaid and Medicare knowledge.
