Job Title: After Hours Nurse Hospice Specialist

Job Summary:

The After Hours Nurse Hospice Specialist is responsible for providing end-of-life care services to patients in their homes or communities. This role requires a skilled nurse to carry out physician orders, assist in planning patient care, and communicate effectively with the healthcare team.

Key Responsibilities:

  • Implement hospice care philosophy and standards in accordance with state and federal regulations.
  • Carry out physician orders accurately and efficiently.
  • Provide skilled nursing care to patients, including wound dressing, enemas, and catheterizations.
  • Communicate patient needs and care plans to the healthcare team, adhering to HIPAA guidelines.
  • Provide basic patient care, including medication administration and documentation.
  • Gather clinical findings and report to the RN or MD at the time of patient death.
  • Instruct patients' families or caregivers on basic care, medication compliance, and other activities as determined by the IDT.
  • Provide care in a respectful and dignified manner, adhering to the agency's patient rights statement.
  • Implement the Five Rights of Medication Administration: the right patient, the right drug, the right dose, the right route, and the right time.
  • Observe and document nursing measures, medication administration, and changes in patient condition.
  • Collaborate and communicate effectively with the healthcare team to establish and maintain cooperative working relationships.
  • Advocate for patients' best interests.
  • Participate in IDT meetings as required.
  • Attend all in-service trainings as scheduled.
  • Maintain patient confidentiality at all times.
  • Comply with infection control policies and procedures at all times.
  • Practice safety in the workplace.
  • Keep the Clinical Services Director fully informed.
  • Fluent in the English language.
  • Ability to position self to move about, ascend, descend or traverse, transport, operate, activate, use, prepare, inspect or detect, perceive, identify, recognize, observe, inspect and assess are required. Ability to convey, communicate, speak clearly and exchange information, converse with, discern, and express oneself.

Requirements and Qualifications:

  • Graduation from an accredited Nursing Program.
  • Current Arizona nursing license.
  • Current CPR Certification.
  • Two years of varied clinical nursing experience; home health or hospice preferred.
  • Hospice philosophy of care.
  • Patient's Rights.
  • Safety in the workplace.
  • Knowledge and support of agency's safety and OSHA policies.
  • Understanding of HIPAA Privacy Standards and related agency policies.
  • Demonstrated understanding and compliance with agency policies, including but not limited to documentation, continuing education, work schedules and required HR documents, State and Federal policies and procedures, laws and regulations.
  • Customer Service Principles.

Skills and Abilities:

  • Structure and content of English language including the meaning and spelling of words, alphabetization, rules of composition and grammar.
  • Calculate general math computations.
  • Organization and time management.
  • Good written communication skills.
  • Moderate computer skills in Microsoft Office applications.
  • Answer and refer telephones calls to ensure accuracy and timely communication.

Special Requirements:

  • Ability to operate a motorized vehicle and is auto insurable with minimum coverage as follows: $100,000 bodily injury per person, $300,000 bodily injury per accident and $50,000 property damage and have reliable transportation for work in the community and surrounding areas.
  • Ability to travel within the agency's service area of 1.5 hours distance to/from the home office. Other travel may include occasional overnight trips to conferences etc.
  • On-call hours may be assigned to include nights, holidays and weekends. Hours may be flexible.
  • Participate in and complete all continuing education activities as assigned in a timely manner.
  • Follow all State of Arizona laws for Mandatory Reporting.
  • Adhere to Hospice of Havasu's Standards of Behavior.
