POSITION OVERVIEW
The hospice aide is an essential member of the interdisciplinary team, working under the guidance of a registered nurse to deliver various services that address the personal needs of patients and promote their comfort. The hospice aide is tasked with observing patients, reporting findings, and documenting care to ensure the quality and safety of care provided.
KEY RESPONSIBILITIES
- Attend to personal care needs and respond promptly to patient requests.
- Ensure patient safety and the proper use of medical equipment.
- Provide personal care services, including bathing, oral hygiene, and changing bed linens.
- Assist patients with dressing, undressing, and meal preparation.
- Measure and record vital signs while ensuring proper skin care.
- Communicate patient conditions and significant changes to the Case Manager.
- Support patients with mobility, exercise, and toileting needs.
- Carry out simple procedures as directed.
- Maintain cleanliness and orderliness in the patient's living area.
- Follow documentation and care protocols diligently.
- Engage in quality assessment and performance improvement initiatives.
- High school diploma or equivalent.
- Current and valid CNA certification.
- Ability to comprehend and follow written instructions and document care provided.
- Understanding of hospice care principles and comfort in delivering specialized support.
- Strong references from previous employers.
- Self-motivated with the ability to work independently.
- Empathy for the needs of patients who are ill, injured, or frail.
- Current CPR certification.
- Effective oral and written communication skills.
- Strong organizational abilities.
- Valid driver's license with an insured vehicle in good condition.
