Case Manager

The Case Manager will work as an onsite member of the GIVE team to support participants of community gun-violence and their families. The ideal candidate will have experience working in the social services field and be able to manage a caseload of clients. The Case Manager will be responsible for managing the site's referral resource database, creating relationships with local agencies and service providers, and attending custom notifications and home visits when necessary. The position requires frequent exposure to individuals displaying high-risk/violent behaviors and frequent weekend and night hours.
Responsibilities
\u2022 Manage the site's referral resource database and create relationships with local agencies and service providers
\u2022 Assist to navigate systems including providing court advocacy
\u2022 Attend custom notifications, and make home visits to participants and families when appropriate
\u2022 Maintain a sufficient caseload of clients at any given time
\u2022 Adhere to all documentation and database requirements and accurately track work
\u2022 Ability to work flexible hours (evenings and weekends) when necessary
\u2022 Any other relevant duties as assigned
Working Conditions/Environment
The position requires frequent exposure to individuals displaying high-risk/violent behaviors, frequent weekend and night hours, and frequent travel within the City of Rochester to different sites. The position also requires automobile, driver's license, and insurance.
Requirements
\u2022 Associates degree from an accredited university
\u2022 Experience working in the social services field
\u2022 2+ years of experience working in or near the SNUG target areas
