Job Summary

We are seeking a highly skilled and compassionate Intensive Case Worker to join our Refugee Services Team. As a key member of our team, you will provide comprehensive case management services to refugee and immigrant clients with complex needs, ensuring their access to essential community resources and promoting their self-sufficiency.

Key Responsibilities
  • Provide culturally sensitive case management services to refugee and immigrant clients, including assessment, planning, and coordination of services.
  • Link clients to a broad range of community-based services, including healthcare, education, and social services.
  • Develop and maintain strong relationships with clients, service providers, and community partners to ensure effective collaboration and service delivery.
  • Monitor and coordinate the delivery of required assessments and services, ensuring compliance with program guidelines.
  • Provide ongoing cultural and community orientation to promote independent access to community resources.
  • Secure and schedule appointments for clients with service providers, arranging transportation as needed, and monitoring medical and health-related goals.
  • Participate in meetings with service providers to increase collaboration and improve outcomes for clients and the program.
Requirements
  • Minimum of an associate degree in a human services-related discipline.
  • Experience providing direct services to refugees and immigrants, with a focus on case management and community resource linkage.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficient in Microsoft Outlook and Word, with experience using electronic databases and case management software.
  • Fluency in a refugee language (Arabic, Burmese, Dari, Pashto, Rohingya, Swahili, Spanish, French, Tigrinya, Ukrainian, or other) is preferred.
