Job Summary

We are seeking a skilled Mental Health Skill Building Specialist to join our Integrated Health Care team. The successful candidate will work with individuals to overcome mental health barriers and achieve their goals.

Key Responsibilities
  • Conduct person-centered assessments to identify client goals and develop comprehensive treatment plans.
  • Provide one-to-one supportive services to individuals in various settings, including on-site and community-based programs.
  • Document client progress and maintain accurate records in accordance with program and agency standards.
  • Collaborate with interdisciplinary teams to provide input and support in the development of comprehensive treatment plans.
  • Assist clients in advocating for their needs with healthcare providers and provide referrals for needed services.
  • Complete all necessary written reports, statistics, and case files in a timely manner.
Qualifications
  • High School diploma or equivalent and at least one to three years of relevant experience working with individuals with Substance Use Disorders and/or Serious Mental Illness.
  • Strong organizational, engagement, and interpersonal skills.
  • Ability to work independently and with flexibility in days and hours of work.
  • Valid NYS Driver's license and access to reliable transportation.
