Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at La Causa. As a Care Coordinator, you will play a vital role in coordinating services for children with severe emotional problems and their families.

Key Responsibilities

Some of the key responsibilities of this role include:

  1. Facilitating all aspects of services for participants, including transportation, assembling child and family teams, assessing needs and strengths, developing individualized Plans of Care, advocating, and providing or securing services as needed.
  2. Serving as a liaison between the child/family and community service providers to ensure the acquisition of services.
  3. Monitoring and negotiating the provision of services, modifying Plans of Care as needed, and submitting required documents by due dates.
  4. Establishing and maintaining good communication, collaboration, and cooperation with all stakeholders, including BMCW, probation workers, and other community partners.
  5. Implementing and practicing the duties and responsibilities of a mandated reporter of suspected child abuse under current policies and procedures.
  6. Attending all studies, meetings, and professional development activities as directed.
  7. Performing other duties as assigned.
Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree in Social Work or a related field.
  • One year of continued experience providing mental health services.
  • Bilingual skills in Spanish and English, required for bilingual positions.
  • Strong cultural competency and interpersonal relationship skills.
  • Excellent critical thinking and problem-solving skills.
  • Highly developed organizational skills and ability to manage multiple priorities.
  • Knowledge and experience with Microsoft Office suite.
  • Valid Wisconsin Driver's license and ability to meet La Causa's driving standards.
  • Ability to work flexible hours, including evenings and weekends, as needed.
  • Successful completion of all background checks.
Working Conditions

This role will require working in field and office environments, including travel. Local travel is required, and some state-wide travel may be necessary. Flexible hours are required, as dictated by program needs.

Salary

The salary for this role is $38,159.44.
