About the Position

We are seeking a skilled and compassionate Nurse Navigator, Surgery to join our team at GW Medical Faculty Associates. As a key member of our thoracic surgery team, you will play a critical role in coordinating referrals, providing education, and facilitating smooth transitions for patients undergoing lung cancer screening.

With a focus on reducing readmissions and improving patient outcomes, this role is ideal for a dedicated nurse with a keen interest in public health and patient advocacy. In addition to your nursing background, you will have experience working in an outpatient setting, excellent time management skills, and the ability to work effectively in a fast-paced environment.

About the Role:

  • C Coordinate referrals and communicate with patients, families, and healthcare providers to ensure seamless transitions
  • Educate patients on their diagnosis, treatment options, and follow-up care requirements
  • Work collaboratively with multidisciplinary teams to develop comprehensive care plans and reduce readmissions
  • Provide support and resources to patients and families affected by lung cancer
  • Foster strong relationships with referring physicians, radiology staff, and other healthcare providers to optimize patient care
