Position Overview:


In a collaborative environment, the Ambulatory Care Pharmacist delivers specialized pharmaceutical care services, adhering to established protocols while working alongside healthcare professionals. This role encompasses the management of patient medication therapies, patient education initiatives, and the oversight of medication preparation and dispensing.

The pharmacist is dedicated to promoting high-quality, evidence-based, and cost-efficient pharmaceutical care within designated clinical settings.

Key Responsibilities:

  • Collaborates with the Department manager or designated personnel to advocate for rational and economical drug therapy in both inpatient and outpatient settings, engaging closely with physicians and medical staff in the decision-making process.
  • Guides and mentors Pharmacist Residents, Interns, Pharmacy Technicians, and other support staff.
  • Conducts thorough patient medication history assessments upon referrals from healthcare providers.
  • Develops comprehensive treatment plans based on diagnoses, incorporating drug therapy and lifestyle changes while ensuring smooth transitions across care settings.
  • Identifies, addresses, and prevents medication-related issues, including untreated conditions, inappropriate drug choices, dosage errors, and adverse drug reactions.
  • Initiates, modifies, monitors, and recommends adjustments to drug therapies in accordance with established protocols, utilizing information gathered from patient interviews and medical records.
  • Oversees and interprets laboratory monitoring related to drug therapies, including serum drug levels and other relevant tests.
  • Engages in patient monitoring to assess and enhance therapeutic outcomes, medication use, and adherence, while minimizing adverse drug reactions and unnecessary medication use.
  • Provides education to patients regarding their health conditions, including expected outcomes, side effects, compliance importance, and specific instructions.
  • Ensures continuous improvement in the quality of pharmaceutical care services provided.
  • Delivers educational presentations and resources to healthcare providers and staff through various formats, including newsletters and individual interactions.
  • Performs additional duties as necessary.

Qualifications:
Experience

  • Completion of a clinical pharmacy residency or equivalent clinical experience in direct patient care.
Education

  • Bachelor of Science in Pharmacy or Pharm.D. degree required.
Licensure, Certification, Registration

  • Active Pharmacist License (California) required upon hire.
  • Basic Life Support certification from the American Heart Association.

Additional Qualifications:

  • In-depth knowledge of pharmacological principles, medical terminology, disease states, and relevant information for effective medical management.
  • Proficient in aseptic techniques and the preparation of sterile IV products and chemotherapy agents, with knowledge of emergency medications and drug monitoring.
  • Familiarity with drug utilization reviews and evaluations.
  • Understanding of federal and state regulations governing pharmaceutical services.
  • Exceptional verbal and written communication skills.
  • Proficient in pharmacy computer systems and related equipment.
  • Must obtain an NPI number within one month of employment.
  • Ability to work effectively in a Labor/Management Partnership environment.
  • ACLS certification may be required in specific areas.
