Key Responsibilities:

  • Collaborate with the Department manager or designated personnel to advocate for rational and economical drug therapy in both inpatient and outpatient settings, actively participating in the medication decision-making process alongside physicians and other healthcare providers.
  • Provide guidance and mentorship to Pharmacy Residents, Interns, Technicians, and other support staff.
  • Conduct thorough medication history assessments upon referrals from healthcare providers.
  • Develop comprehensive treatment plans based on diagnoses, incorporating drug therapy and lifestyle changes while ensuring continuity of care.
  • Identify, address, and prevent medication-related issues, including untreated conditions, inappropriate drug choices, dosage errors, and adverse drug reactions.
  • Initiate, modify, monitor, and recommend adjustments to drug therapies in accordance with established protocols, utilizing information gathered from patient interactions and medical records.
  • Oversee and interpret laboratory tests related to drug therapy, including serum drug levels and other pertinent evaluations.
  • Monitor patients to evaluate and enhance outcomes, drug utilization, and adherence, while minimizing adverse drug reactions and unnecessary medication use.
  • Educate patients about their health conditions, including expected effects, side effects, the importance of adherence, and any specific instructions.
  • Ensure continuous improvement in the quality of pharmaceutical care services provided.
  • Deliver educational sessions to healthcare providers and staff through presentations, newsletters, and individual discussions.
  • Perform additional duties as necessary.

Qualifications:
Experience

  • Completion of a clinical pharmacy residency or equivalent clinical experience in direct patient care.
Education

  • Bachelor’s degree in Pharmacy or Pharm.D. required.
License, Certification, Registration

  • Valid Pharmacist License (California) required upon hire.
  • Basic Life Support certification from the American Heart Association.

Additional Requirements:

  • In-depth knowledge of pharmacological principles, medical terminology, disease states, and related information for effective medical management.
  • Proficient in aseptic techniques and the preparation of sterile IV products and chemotherapy agents, along with familiarity with emergency medications and drug monitoring.
  • Understanding of drug utilization reviews and evaluations.
  • Knowledge of relevant Federal and State laws and regulations governing pharmaceutical services.
  • Excellent verbal and written communication skills.
  • Ability to operate pharmacy computer systems and related equipment.
  • Must obtain an NPI number within one month of employment.
  • Ability to work effectively in a Labor/Management Partnership environment.
  • ACLS certification may be required in certain areas.
